Refer Save Apply Job ID: DC-42655 Description: This is a grant funded position.Clinical PracticeProvides a consultative role in the management of geriatric patients and will participate as a part of interdisciplinary teams providing care to elders. Will be actively involved in developing and implementing geriatric best practice protocols and consequent interdisciplinary educationAssures patients and families are kept informed of the clinical management care plan and clinical outcomes on a daily basis.Collaborate with physician to ensure patient and family needs are met.Participates in formal and informal patient teaching activities aimed at promoting positive health practices and an understanding of the individual's medical problems Confers with physicians regarding assigned patients keeps physicians informed as is appropriate to the needs and problems related to patient care.Will be available for consultation on the care of complex geriatric patients while in patient and/or post discharge via telephone. Will develop and help lead best practice education and training opportunities across the multidisciplinary team across the continuum of care. Leadership Contributes to knowledge development and improved care of the adult-gerontology population.Uses scientific knowledge and theoretical foundations to differentiate between normal and abnormal changes in physiological, psychological, and sociological development and aging. Scientific foundationProvides guidance, consultation, mentorship, and educational experiences to students, nurses, and other health professionals regarding acute and critical care populations.Participates in clinical and nursing research projects.Mentors and or educates nurses on principles of nursing research Analyze data and recommend areas for nursing or clinical research.Serve as mentor and PI for Summer Intern students. QualityImplements evidence-based practice interventions to promote safety and risk reduction for the older adults with acute, critical, and complex chronic illness needs.Develops templates and clinical pathways based on evidenced based guidelines and literature.Develops a system for patient reported outcomes measurement and serves as an expert in the analysis of the data collected.Maintains compliance with state, federal, and JC and other regulatory requirements.High competency with data acquisition and analysis and Lean methodologiesTechnologySynthesizes data from a variety of sources, including clinical decision support technology, to make clinical decisions regarding appropriate management, consultation or referral for acutely and critically ill patients.
