Refer Save Apply Job ID: KMB-66523 Description: The Oncology Patient Financial Navigator provides individualized assistance to patients, families, and caregivers to help overcome health care system barriers and facilitate timely access to care through all phasesThe Oncology Patient Navigator assesses the financial needs of new and established patients related to clinical care according to specified protocol. Provides patient and health care team education, links to resources and data collection in accordance with the process identifiedScreens newly diagnosed patients to identify those who may have financial challengesMeets with patients when diagnosed to provide supportServes as liaison between the treating physician and the patient, especially with regard to financial issuesIdentifies patients that need navigation and assistance with barriers such as lack of insurance, financial issues, need for personal support, language and any other problems that may arise during treatmentProvides health education and community outreachWorks in conjunction with all members of Oncology, medical staff, and administration to provide quality comprehensive care to patient, and ensure that barriers to treatment are overcome.Provides Psychosocial support for cancer patients; one on one and group teaching sessions as feasible and appropriateAssists with access issues and referral process for insurance and financial forms. Arranges transportation when needed.Assists and empowers patients to interface and negotiate with their health insurance providersParticipates in activities to enhance professional developmentPerforms these services throughout the continuum of care (outpatient, inpatient and other various departments)Participates in data collection for quality and research purposesCoordinates and plans financial navigation conferenceRequired Minimum Experience: Two years clinical experienceRequired License/Certification: Current license as a Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker-Clinical (LCSW-C) from the Maryland Board of Social Work ExaminersLight Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
