JOB SUMMARYThe Case Manager willprovide intensive case management services and support to Veterans and theirfamilies who may have complex needs. He/she will assist Veteran families withaccessing the community resources necessary to meet their individualizedpermanent housing needs including, but not limited to: mental health, medical,financial, educational, vocational, residential and social supportservices. He/she monitors and documentsconsumer progress and outcomes. ESSENTIAL DUTIES Utilizes counseling and communication skills to engage Veteran families and build positive working relationships Acts as liaison between the Veteran, Project PLASE and relevant community-based organizations, services, health facilities, etc. Conducts community-based outreach and intake to identify and market services to eligible program participants Completes an initial service plan within 5 days after initial assessment and update the service plan at least every 3 months Implements service plan Establishes a file for each client that includes all documents, contacts and information related to the individual Actively provides linkages to community agencies Provides transportation for clients to various appointments, meetings and functional/leisure time activities Conducts Veteran assessments, evaluations, complete forms, narratives, referrals, process/create and implement service plans/agreements Assists client in obtaining and maintaining all entitlements and benefits for which they are eligible for (ie VBA, HUD-VASH, MA, SSI and/or SSDL, Section 8 housing, Pharmacy Assistance, unemployment, alimony, TANF, food stamps, etc.) Requests financial assistance according to the DVA federal rule to help Veterans meet their goals and maintain housing stability Works as part of a team with other PLASE, HHVF staff, interns and volunteers Works closely with Peer Veteran Advocate Helps clients and/or family to continue to grow, meeting goals and prevent further homelessness Attends continuing education courses, workshops and seminars Performs other job related duties as assigned MINIMUM QUALIFICATIONSEducation andExperience: Masters' degree in socialwork or human services; plus, 1 year of experience as a social worker or humanservices professional.CERTIFICATIONS AND LICENSESApplicants must have a valid drivers' license to operate avehicle in the State of MarylandPHYSICAL AND ENVIRONMENT CONDITIONSWork requires light physical effort in the handling of lightmaterials or boxes in non-strenuous work positions of up to 30lbs. and/orcontinual standing or walking Work environment involves everyday risks or discomfortswhich require normal safety precautions typical of such places as offices,meeting and training rooms, residences or vehicles (eg use of safe work placepractices, avoidance of trips and falls, observance of fire regulations andsafely operating a motor vehicle.How to apply: Please send resume'/application to: Sheri L.
