Works part-time at the Merrifield center with Fairfax-Falls Church Community Services Board. Provide case management, case coordination and outreach services to individuals in Detox (individuals on the waiting list for detox who never get in due to lack of follow through.) Additionally, works with individuals leaving residential treatment programs against staff advice, individuals in outpatient detox, individuals at the AIB /jails, individuals in shelters and individuals being discharged from local hospitals. Provides aggressive outreach services to include personal phone calls, visits to those homeless shelters or attending homeless drop-ins. Provides continuous triage to include assessment of individual's current level of functioning, mental status, and level of risk/immediate needs. Documents all client contact and services in EHR according to program licensure methods and standards Completes appropriate forms and procedures to use for completing all clinical paperwork in a timely manner, according to agency, state and licensure requirements.Employment Standards
Graduation from an accredited college with at least a bachelor's degree in social work, psychology, sociology, counseling, nursing (or, for certain positions providing services to children and adolescents, a bachelor's degree in special education) plus two years of clinical experience or graduation from an accredited college or university with a master's degree in social work, psychology, sociology or counseling plus one year of clinical experience. Clinical experience means providing direct clinical services to individuals with mental illness and includes supervised internships, practicum, and field experience. Additional education may not substitute for clinical experience.
First Aid and CPR certification, within 90 days of appointment and maintained throughout CSB employment, for positions in residential programs. Medication Administration Assistance Certification within 90 days of appointment, for positions in residential programs. Valid driver's license. Annual TB test.
NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, an Office of the Inspector General (OIG) check, a check of the Child Protective Services Registry, and a driving record check to the satisfaction of the employer.
Observe process and document clinical information in electronic health record; make appropriate clinical interventions.
MA degree in social work or related field and at least two years experience providing services to substance use dependent clients. Licensed or license eligible.
