Clinical Housing Coordinator
The Veteran's Supportive Housing Team is pleased to announce we have an opening for a Clinical Housing Coordinator serving individuals and families with histories of chronic homelessness, serious mental health, addiction and trauma, and many of whom have served in the military. The Clinical Housing Coordinator builds trusting relationships with program participants to assist them with establishing and maintaining housing, enhancing income, and promoting self sufficiency. Required Qualifications: BA in related human services field Preferred Qualifications: 2 years experience working with underserved individuals and families with complex needs; Commitment to preventing homelessness and utilizing a Housing First approach; Some exposure to supporting Veterans and their families; Strong clinical and administrative skills; Exceptional care coordination and teaming practices. Specific responsibilities include: Monthly home visits with participants, Comprehensive housing assessment, Services planning, Coordination of care, and Direct delivery of services to promote housing and income stability (e.g., skills building, resources acquisition, service referral and connection, etc).
