Currently seeking an experienced Hospice Registered Nurse Case Manager for a full-time position in the Washington DC region . Position Pays $75k - $82k annually. Title: Hospice (RN) Case Manager Reports To: Clinical Manager Supervises: LPNs, CNAs, HHAs Summary and Responsibilities: A clinical professional responsible for ensuring quality total patient care through assessment, planning, interventions, and evaluation of patient needs through nursing care plans for each assigned patient. Under the direction of the physician, will address patient and family needs; use evidence based practice; directs Licensed Practical Nurses, Certified Nursing Assistants, and Home Health Aides in quality patient care; provides appropriate patient/family/caregiver teaching; promotes quality services in accordance with established agency policies and procedures; ensures compliance with federal, state, and local guidelines for all services; meets budgeted productivity standards; assures the quality and growth of the agency and; participates in the education of facility and community healthcare systems. 1. Performs initial and ongoing comprehensive assessments 2. Acts as case manager by coordinating all patient/family services and prioritizing the patient needs with the members of the interdisciplinary team 3. Applies specific criteria for admission and re-certification to hospice care, as applicable, to establish appropriate levels of care and patient's eligibility 4. Initiates appropriate palliative interventions/symptom management and monitors effectiveness; provides referrals to other services and disciplines as needed 5. Accurately and completes all clinical documentation per agency policy 6. Recognizes the importance of patient privacy, uses the secure messaging system for any patient information 7. Performs accurate medication review and provides education as needed 8. Assesses for medication, equipment and medical supplies requirements and orders appropriately while being fiscally responsible 9. Identifies and communicates changes in the patient's condition; informs the physician and obtains new orders as needed 10. Maintains communication with the patient, family and other members of the home care team to ensure coordination of services and clinical management that meets patient and family goals in a cost-effective manner 11. Evaluates outcomes of care through performance improvement activities, clinical record review, and other assigned duties 12. Attends the two weekly interdisciplinary meeting, actively contributes to the patient care discussion, ensures multidisciplinary involvement, updates the plan of care as appropriate 13. Supervises LPN/s and paraprofessionals providing hospice services for the agency 14. Participates in staff meetings and at least quarterly, in-service programs 15. Coordinates the discharge from service if appropriate or bereavement follow-up after hospice death 16. Responsible for interpretation and compliance application of all federal and state regulations Supplemental Duties and Responsibilities: 1. Participates in the on-call rota as required 2. Provides daily report to Clinical Manager as necessary 3. Participates in personal and professional growth and development a) Responsible for providing positive model for team through effective self-management and leadership b) Maintains current knowledge of industry guidelines c) Maintains clinical skills and competency to be a resource to staff d) Seeks additional training/education as determined by self-assessment and manager 4. Fulfills additional performance responsibilities as assigned and as necessary a) Willingly accepts additional job responsibilities, as appropriate, to meet business goals/objectives b) Demonstrates flexibility in order to maximize patient care and productivity Qualification Requirements: 1.
