Licensed Social WorkerCommunity Solutions, Inc, is looking for a dynamic Licensed Social Worker to join our very successful team. Our TFC program is focusing on the transitional age youth in Maryland. The main goal is to help these youth integrate into the community and live independently of the child welfare system. Position is assigned to the Towson, MD Office but serves within and outside the Baltimore Metro Area.Position Qualifications:* Masters Degree in Social Work or related field* Graduate Level Licensure (LGSW)* 3 years work experience* LCSW or LCSW-C preferredCompany Benefits:* 2 weeks vacation,3 weeks after 1 year* Medical, dental, vision, life insurance* 401k participation with up to a 4% company match* Company support for additional training and developmentPosition Responsibilities:Supervision Training and Support* Directly supervise all activities of treatment parents related to the treatment and care of TFC youth through regular home visits and phone calls* Provide in-home training as needed for TFC parents* Provide 24-hour emergency "on-call" service to TFC families according to designated schedule* Assist in the arrangement of respite servicesDocumentation and Compliance* Perform duties in compliance with COMAR regulations.* Maintain documentation of in-home and other ongoing training activities* Prepare written summaries, Individual Service Plans and quarterly updates, discharge summaries, and program contract regarding youth and families* Ensure completion of and collect TFC parent documentation, Medication Logs, Respite logs, copies of medical information, and any other required documentationTreatment Planning and Service Delivery To assure the development and implementation of educational and treatment plans at school, home, and in the community* Assess each child's needs in all life domains and develop measurable and objective treatment plan goals* Provide ongoing advocacy and support for the child's educational needs* Initiate and coordinate psychiatric, psychological, and other supportive therapeutic services when appropriate* Monitor the progress of the child's goals and modify the treatment plan as needed* Provide overall leadership and coordination of the treatment teamSafety & Well Being* Assure that all policies, procedures and standards relating to child safety and well-being are followed* Make regular contact with each youth to assess safety and well being* Assure that TFC parents follow procedures for child illness and provision of routine healthcare* Develop an individual safety plan for each child and assist other staff, in crisis intervention to protect youth, staff, and property when transporting youth, maintain safe driving procedures* Adhere to mandated child abuse reporting lawsWorking with Biological Family To maximize the likelihood of a positive relationship between TFC youth and their biological families* Empower birth families through systems education and advocacy training* Involve the biological family in all aspects of treatment planning including regular updates on youth's progress* Coordinate regular contact between the child and his/her biological family as appropriateCommunication & Community Resources to assure continued communication between the programs, the child's family, relevant community agency and case aides* Develop each youth's treatment team including formal and informal supports as identified by the youth, family, and guardian* Maintain regular contact and open lines of communication with youth's treatment team and any other significant community persons* Represent agency regarding court involvement of the youth* Link youth and family to appropriate pro-social activities that foster success and healthy development* Respond to all complaints/concerns from TFC parents in a timely supportive mannerCommunication & Community Resources to assure continued communication between the programs, the child's family, relevant community agency and case aides* Confirm youth have completed an independent living assessment for life skills* Submit assessment to the appropriate individuals for scoring* Supervise any program aides that are assigned to the youth to assist in the implementation of the treatment and service plan* Work in collaboration with other service providers to develop and implement intervention procedures in the homes working toward achievement of the permanency plan* Coordinate all community resources pertaining to discharge placement* Arrange aftercare follow up if indicatedProfessional Development* Actively participate in individual and/or group supervision and staff meetings* Attend and participate in all required and other professional trainings* Independently seek out resources and learning opportunities to foster ongoing professional growthPosted by StartWire.
