HomeCare Maryland is seeking a Physical Therapist PT with home care experience. HomeCare Maryland is located in Baltimore Maryland and serve clients I in the Baltimore City and Baltimore County areas. 1.Complete the initial assessment / evaluation visit and regularly reevaluates the client's care needs.2.Initiates or reviews the plan of care and makes necessary revisions.3.Provides or delegates those services requiring substantial and specialized therapy skill.4.Initiates appropriate preventive and restorative therapy procedures.5.Prepares clinical and progress notes and summaries of care.6.Counsels the client and family in meeting related needs. Qualifications: *Minimum of Bachelor of Science degree in Physical Therapy.*Licensed Physical Therapist in the state of Maryland*Minimum three (3) years of clinical experience with at least half of that time working in home health or related health care organization. Demonstrated ability to supervise and direct staff.*Possess exceptional therapy assessment skills*Proven ability to build quality and implement improvements within the operation.*Excellent interpersonal skills.*Analytic/Critical thinker.*Certification and demonstrated competency in identified areas of specialization.*Demonstrated competency in applying the principles, methods, materials and equipment used in physical therapy.* Ability to assess resident needs and develop and implement a comprehensive plan of care.* Knowledge of accreditation standards and compliance requirements.
