Minimum Education, Licensure and Certification: Case Manager (Supportive Counselor) must currently have a minimum of 60 credits in a Human Service field; or at least 2 years field experience in a Human Service environment Note: This position requires reliable transportation and individuals must be willing to service in home Report to: Program Director Duties and Responsibilities: In general, the provision of sound, best practice models of direct clinical services to women, and families:Treatment (of Mental Disorders/ Psychiatric Conditions) Conduct individual, family and group training through skills building exercisesDevelop implement and evaluate progress towards goals and objectives of each clientProvide treatment only in accordance with what has been prescribe under the current treatment planRespond to crisis situation and deescalate, as necessaryAssist clients with adopting available community resourcesAid in the delivery of the required program services including, meeting the psychosocial needs of participantsCoordinate and arrange for social-skills building activities in the community, including linkage with self-help groups, churches, recreational and other programs that will encourage clients to follow treatment recommendationsDocumentation Document each encounter with clientSubmit all billing documentation on timeReportsMonthly reports of case management engagement, activities and progress toward goals and objectivesComplete Concurrent Reviews every (3) months for children and (6) months for AdultsOther reports as specifically requested by CEO, COO, CD or PDMandatory Meetings Attendance.Meet with the Program Director to review, plan and prescribe most effective course of treatment for the clientAttend a mandatory training and development meeting once a month pertaining to understanding the overall culture and operations of the program and organizationSupervision and Training Attend in-service training and/or continuing education as required for this positionOther Possess good verbal and written communication skillsPossess knowledge of and adherence to ethical standards in clinical practicesMaintain a good understanding of the culture and current policies and procedures of Her Mind Her Body Wellness ProgramComply with policies and procedures, Licensing Board and other requirementsClinical supervision offered, must meet qualificationsRequired to do the minimum amount of encounters according to COMAR regulations monthly for both adults and children in order to receive maximum payout for clients on your caseload.Possess the ability to participate in the fitness and wellness component of the program Salary: This position is fee for service (paid per client visit): $20 - 27, but not limited to, experience and academia Applicants must be able to service a minimum of 10 clients. However, the option to service a full case load will depend on one's level to demonstrate their ability to handle such responsibilities.
