Required education: Bachelor's, Master's degree PRP Learning Center of Baltimore is seeking a licensed social worker or professional counselor to serve as Rehabilitation Specialist. The ideal candidate will have, at a minimum, licensure as LGSW or LGPC and at least two years' experience as a clinician. Duties include intake assessments, authorizations in Beacon Health system, development of treatment plans, assisting Program Director and Coordinator in staff and personnel hiring and training. Organization and timely documentation are critical factors in the position. Salary is commensurate on level of licensure, experience in the field, and dedication to document management Minimum Requirement: The Rehabilitation Specialist must be a Licensed Mental Health Professional (LCSW, LCSW-C, LCPC) with a minimum of two years' experience working with mentally ill.The Rehabilitation Specialist must be able to demonstrate proficiency in psychiatric rehabilitation assessments and Individual Rehabilitation Plans (IRP).The Rehabilitation Specialist must be able to educate, monitor, supervise and support direct care staff in the Adult PRP.The Rehabilitation Specialist should have experience conducting individual and group psychotherapy for adults and experience planning therapeutic group activities.Excellent communication skills and excellent written skills. Duties and Responsibilities: Lead and coordinate Individual Rehabilitation Program team meetings with engaged stakeholders, including therapists and consumers.Develop Individual Rehabilitation Plans for each consumer, with domain, short and long-term goals and interventions.Advising the program director and managing with Program Coordinator regarding development and implementation of rehabilitation services that reflect the need of the clients served by the program.Meet with Psychiatrist once a month for coordination of members care.Conduct PRP intakes and obtaining all necessary signatures for appropriates paperwork of minors starting PRP.Obtain initial PRP authorizations and educate staff on how to provide rehabilitation services that correlate with goals.Reinforce company policies; COMAR regulatory requirements, and other regulatory agency requirements.Review and sign client's contact notes/ monthly progress notes and record in client tracking recordEducate direct service staff on how to provide rehabilitation services that correlate to the goals identified in the individual's IRP.Conduct staff meetings weekly for planning and assessment of staff development.
