Family Nurse Practitioner (FNP ) - Direct Patient Care - York/Lancaster PA areas to see company patients. The volume supports one NP to work approximately 2 days per week through the end of 2017. Much of this volume will be well child and adolescent visits - would like to assign providers that have afternoon/evening and weekend availability. FAMILY NURSE PRACTITIONER JOB DESCRIPTION Perform health risk assessments which are essentially history and physicals done within the within the patient's home or on occasion in nursing home or other health facilityWill see adults, children, and/or postpartum visits depending on needRequires clinical documentation of acute, chronic, and long-term healthcare issues through patient encounter in EMR system Documentation must be accurate, high quality, and completed in a timely mannerWill ensure all appropriate consent forms are signed and datedWill have frequent interaction with patients, coders, schedulers, and supervising physicianEducate patients and/or patient's family on chronic medical conditions, preventative care, and medication adherence complianceComplete referrals to case management via our EMRParticipate in clinical meetings and compliance training's which are primarily web based; however, NP will occasionally be required to attend on-site training for which will be reimbursedWill not be required to prescribe medicine nor order any lab or diagnostic testingWill occasionally need to obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine collection/analysis may also be required based on client need.Be responsible to review patient records for past and present health issues and collaborate with the supervising physician and other members of health care teamMust be able to interpret laboratory data and diagnostic testing when available to assist in diagnosis of medical conditions As part of the assessment, must do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation.If NP arrives at the home of a patient and the patient is not home and does not answer the phone, will immediately call the office and inform the office of the patient's unavailabilityDocument all conversations with the patients in the patients' chartRequired to inform the office at least one (1) week in advance of a vacation or a personal absence that is planned for two (2) or more consecutive days so the office can arrange for the scheduling of patients Required to inform the office at least one (1) month in advance of a vacation or personal absence that will extend beyond one (1) week so the office can arrange for the scheduling of patients Required to notify the office within four (4) hours of the Provider's first scheduled patient appointment if unable to report to workWill be expected to schedule at least six (6) visits per day while maintaining an average of number of patients seen of four (4)per day.Should a provider consistently fail to meet theses expectations then at the discretion of the supervising physician the provider may be prevented from seeing additional patients. SKILLS/REQUIREMENTS FOR NP POSITIONExperience with electronic medical records; must be proficient within one of the well-known EMR systems within the medical community.Position requires utilization of several different technologies (smartphone, laptop, etc.) so again only very computer proficient individuals are encouraged to apply.Proficiency with Microsoft outlookMust be comfortable doing postpartum visits which are essentially an adult history and physical with additional postpartum related history takingStrong background/interest in primary care and preventative medicine is a plusMust have valid Pennsylvania driver's license, provide your own vehicle, have proof of adequate automobile insuranceHighly motivated, self-directed professional with strong organizational skills and comfortable working independently on a daily basisExcellent clinical assessment and analytical skillsEffective leadership and interpersonal skills; facilitates problem resolution and maintains a professional demeanor in difficult situations.Excellent verbal and written communication skillsProven ability to perform comprehensive, adult/geriatric/pediatric/postpartum physical exams Demonstrated ability to identify, diagnose and document chronic disease in adultsDemonstrated ability to recognize conditions, characteristics, and behaviors that place seniors and children at riskAbility to pursue feedback, continuing education and professional development Candidate must be willing to travel up to a 100 miles radius from their home zip codeMust be familiar with preventative medicine in particular cancer screening recommendations Problem Solving Team Work Dependability Attendance/Punctuality Initiative Judgment and decision making Superb medical knowledge of the information and techniques needed to diagnose and treat injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health care measures.Strong reading and oral comprehension -English language required.
