The Therapist/Counselor conducts the Clinical Assessment and develops the initial Treatment Plan, and the Targeted Case Management Plan. The Therapist/Counselor provides individual and family therapy to assigned clients. This position has a great deal of flexibility and does not require the employee to be in office. The majority of the work will be performed in the field. Family Preservation Services (FPS) is a subsidary of Pathways. Position is in West Virginia ESSENTIAL JOB FUNCTIONS AND REQUIREMENTS *Conduct clinical assessments on clients, and prepares assessment report in timely manner*Develop initial treatment plan for supportive counseling services with client and client's support system*Develop therapy plan for therapeutic services with client*Provide individual, family, and group therapeutic sessions and interventions as outlined in the therapy plan, using a strengths-based approach and best practice models*Establish and maintain a therapeutic relationship with the client and the client's support system, with an awareness of maintaining appropriate boundaries*Document all services provided to the client and about the client to include all billable services and non billable services according to FPS and Medicaid standards. Complete and submit documentation within one week of date of service*Consistently submit time sheets and related paperwork on time every pay period*Provide crisis intervention services to clients as necessary*Attend and cooperatively and constructively participate in treatment team, supervision meetings, and required training.*Treatment team meetings can include: staffing, evaluation of intervention, identification of needs, review of monitoring and linking of community based resources for the individual and family*Provide information for court reports and other client related reports as required, and attends court when subpoena*Consistently maintain established productivity requirements as dictated by case assignments*Serve as an FPS Ambassador by positively representing FPS in working with clients and families and in working with community partners, referral sources, etc*Implement and manage Safety Plans with client and client's support system as needed*Locate, link, and refer client and client's support system as necessary to applicable community resources, including child-oriented activities*Maintains compliance with all requirements of CFIT*Maintain the ability to transport self to clients' homes and other community locations which requires maintaining a valid driver's license and safe driving record and adequate, current car insurance.*Cooperatively perform other job duties as assigned to support the provision of quality services to clients and FPS's mission QUALIFICATIONS *Possession of a Master's degree from an approved college or university in a Human Services discipline BENEFITS *Health, Dental and Vision insurance, 401K plan, paid holidays and paid time off (PTO) Company Description: Pathways mission is to ensure the provision of accessible, effective, high quality community-based counseling and social services as an alternative to traditional institutional care.
