This position is located at 7215 York Road, Baltimore, MD 21212. The work schedule is Monday through Friday 8:30 am - 4:30 pm. POSITION SUMMARY: The Registered Nurse Case Manager is responsible for enhancing the outcomes of persons supported in The Arc of Baltimore Community Living Program. The RN Case Manager will proactively address any health and medical needs, act in accordance with the agency's mission, in compliance with the MD Board of Nursing (MBON) and the Developmental Disabilities Administration (DDA) regulations. This Nurse will frequently teach the Direct Support Professionals about the consumers' medical and nursing care needs and delegate much of the nursing care to the Direct Support Professionals, in accordance with the DDA and MBON regulations. The RN Case Managers will be supervised by the Director of Nursing. DUTIES & RESPONSIBILITIES: + Schedule and provide all Nursing Reviews on time and as necessary to meet needs of persons supported and State of Maryland Regulations. + Comply with and remain abreast of all Board of Nursing and COMAR Regulations as they apply to position. + Communicate effectively with all Arc of Baltimore Staff, Intergovernmental or Interagency Staff. + Complete Initial Nursing Assessment and Plan of Care when new consumers are placed on a RN Case Managers' case load and review these at least annually or when significant changes occur. + Update the Plan of Care for assigned caseload as necessary. + Review appointment records, laboratory and diagnostic tests on respective case load. + Support, teach, and appropriately communicate with the Direct Support Professionals about the consumer's medical and nursing care needs, including IP Specific Trainings. + Delegate appropriate nursing care as necessary and in accordance with the Plan of Care. + Successfully complete RN Case Manager, DDA/ MBON training class. + Perform medication delegation and maintenance of 6 month redelegatons for staff Certified Medication Technicians. + Participate in IP Meeting for consumers on the respective nurse's case load and cover other nurses' case loads as necessary. + Review any medication errors that occur, completing necessary documentation and facilitate more training as necessary. + Report medication errors according to DDA regulations. + Interact with physicians, pharmacy staff to manage medication needs of persons safely and according to DDA and MBON guidelines. + Provide 24-hour pager availability for nursing consultation to Arc of Baltimore employees per rotating on call schedule. + Attend additional meetings as appropriate including: Nursing Staff Meetings, Community Living Staff Meetings, DDA quarterly Nursing Meetings, and Hospital Discharge meetings. + Teach and follow The Arc Baltimore infection control policies. + Follow HIPAA guidelines to ensure confidentiality. + Complete all agency required trainings for position. + Perform other duties as assigned by Supervisor. EDUCATION & EXPERIENCE: + Graduation from an accredited School of Nursing. + RN (Registered Nurse) licensure from the MBON. + BSN (Bachelor of Science in Nursing) preferred. WORKING CONDITIONS: + Some evening and occasional weekend hours may occur. + Required to be on call approximately every 6 weeks for one week between the hours of 4:30 pm and 8:00am the following day during the work week and for 24 hour coverage on weekends and holidays. + Required to provide an On Call reports.
