Clinical Care Coordinator
Clinical Care Coordinators provide integrated care management on a multidisciplinary team serving adults with mental illness and health concerns. This team is called a Health Home. The clinical care coordinator provides community support services to consumers in collaboration with a nurse care manager so that both physical and mental health goals are addressed in an integrated approach. Case ratios are roughly 50 consumers per care coordinator. Clinical Care Coordinators must have interest in promoting the integration of care across mental and physical health domains. The position is community based and tracks a variety of outcomes to determine if consumers are improving their overall health. This position requires a BA degree. Candidates must have a reliable vehicle as much of the work is done in the community.
