THE JOHNS HOPKINS HOSPITAL BALTIMORE, MD Pay Grade: RM The Kidney Transplant Nurse Coordinator is responsible for: a€ cents the coordination and total scope of care for a group of organ transplant recipients (actual or potential) or donors as they navigate though the evaluation of, preparation for, and/or life with a transplanted organ or having donated an organ or partial organ. a€ cents provision of patient care based on the nursing process, mentors others to promote quality and excellence of care a€ cents working collaboratively with and providing leadership for a multi disciplinary health care team to optimize performance a€ cents promotion of improvement in the transplant office and organizational systems to benefit patient care a€ cents incorporating and operationalizing goals and objectives of Hospital at site of patient care a€ cents collection and maintainence of data required for patient care, research and reporting to outside regulatory agencies a€ cents as needed call activities that may include organizing transplants and triaging patient problems a€ cents may assume office management functions as designate by the Nurse Manager Johns Hopkins Health System and its affiliates are an Equal Opportunity / Affirmative Action employers.
