Summary: The Community Health Nurse (CHN) is part of the Hospitala€ (TM) s Community Care Team (CCT). CCT is a program focused on improving the health of complex, chronically ill Howard County residents. This program helps individuals connect with resources that allow them to better manage their illness, improve their health and avoid preventable hospital encounters. CCT helps clients to meet their goals by providing health education, teaching chronic-disease management skills, and by connecting them to a medical home and other community resources. The CHN is an integral part of the multi-disciplinary team that includes Community Health Workers (CHW) and a Licensed Clinical Social Worker (LCSW). The CHN supports the implementation of a care plan and performs essential clinical functions as part of a clienta€ (TM) s care team. This position works across care settings as well as conducts regular home visits. Requirements: A Bachelora€ (TM) s Degree in Nursing from an accredited college or university and a minimum of two years of experience in in public/community health as well as one year of additional experience in an administrative, supervisory, consultative or teaching capacity as a registered nurse. A candidate may substitute a Bachelora€ (TM) s degree in a related field from an accredited college or university and one year of experience as a Registered Nurse for the required education. Experience working in a community setting or home setting preferred.
