The Team Clinical Supervisor is the leader of the patient care team whose function is to: supervise, direct, evaluate, coordinate and support the various members of the interdisciplinary team. The Clinical Supervisor must assure continuity of care from admission to discharge or transfer to bereavement; serve as a staunch patient advocate and coordinator for other social services and health care providers in the community who are involved in the care of the team¿s patients. Must assure responsibility for the maintenance of patient records from intake through discharge and transfer of bereavement. Participates in quality improvement initiatives and guarantees the use of an interdisciplinary approach to assess patient needs.
RN-Bachelor's Level Degree
Required: Hospice - Previous experience, Nursing - 2 years
Preferred: Supervisory - 3 years
Required: Basic Life Support, Registered Nurse

All new RN's who do not have their BSN will be required to sign a BSN agreement committing to enrollment in accredited RN to BSN program within 24 months of hire and successfully obtaining their BSN within 5 years of hire. (Exemption to expire November 30, 2017.)
