APC PROGRAM COORDINATOR-SNGH
Sentara Cardiology is seeking an APC to join their Structural Heart team at SNGH. The APC will perform inpatient consults, rounding and discharges. In the outpatient role the APC will conduct new patient visits and follow-ups. Surgical procedures performed by the APC include caths, cardio versions, TEE. This is a full time, Monday through Friday position with a rotating shift. Call is required.
Provides comprehensive care to a defined population of patients utilizing defined evidenced based protocols and guidelines. Collaborates with care team providers and is innovative in creating an improved patient experience. Provides strategies for streamlining clinic functions and has strong leadership skills to help develop various program needs.
Master's Level Degree - NURSE PRACTITIONER OR
Master's Level Degree - PHYSICIAN ASSISTANT
Required: Nurse Practitioner - Previous experience
Preferred: Physician Assistant - Previous experience
Required: Authorization to Prescribe, Basic Life Support
Preferred: Nurse Practitioner, Physician Assistant, Registered Nurse
Required: Communication, Complex Problem Solving, Judgment and Decision Making, Leadership, Speaking, Writing
Preferred: None, unless noted in the “Other” section below
Licensure: Nurse Practitioner or Physician Assistant is required. Advanced Cardiac Life Support (ACLS) preferred. The APC Program Coordinator (APC) maintains continuing education credits of at least 30 hours per year and maintains prescriptive authority and certification; acquires special clinical competencies as required by the group; participates in quality assurance and utilization management activities; and assists with risk management functions. For those with NP license, RN license also required.
