Center Nurse - Licensed Practical Nurse
This position is located at the Lewinsville Adult Day Health Center.
Under the direction or supervision of a public health nurse, as part of the Interdisciplinary Care Team (ICT), the center nurse (CN) is responsible for providing clinical nursing services and supervision for a specialized population of individuals. Most of the participants are frail elderly with a variety of diagnoses and multiple disabilities served in an Adult Day Health Care (ADHC) setting. Assesses, monitors and documents participant's health status on a regular basis. Develops and updates the participants' individual plan of care with both immediate and long term goals. Provides documentation in chart per agency protocol. Occasionally assists with recreational activities, ADL's and toileting of incontinent individuals. and toileting of incontinent individuals.
Graduation from an accredited practical nursing program; plus two years of experience as a Licensed Practical Nurse. CERTIFICATES AND LICENSES REQUIRED: Possession of current license to practice as a Licensed Practical Nurse (LPN) in the Commonwealth of Virginia. NECESSARY SPECIAL REQUIREMENTS: The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry, and driving record to the satisfaction of the employer. TB Test required. PREFERRED QUALIFICATIONS:At least two years' experience working on a multidisciplinary team serving older adults with cognitive and physical impairments. Experience conducting health assessments, developing care plans and managing chronic disease. Experience with health education of clients and caregivers. Fluency in English with bilingual skills in Spanish or Korean.
PHYSICAL REQUIREMENTS:Required to walk, stand for long periods of time (i.e.: 1 to 2 hours), bend, sit, kneel, reach, climb stairs and participate in recreational therapy. Able to assist with client transfers from sitting to standing and lift up to 30 pounds. Uses hands to grasp, handle or feel. Visual acuity is required to read data on a computer monitor.
