Position Purpose:General Summary:

Instructor who has completed four-day lay leader training for Stanford University Chronic Disease Self-Management Program. Responsibilities include facilitating six-week workshops as assigned and adhering to program fidelity. Supports Mission of Trinity Health and Holy Cross Health.

Minimum Licensure & Certification Required (if applicable):

1. Must have completed four-day lay leader training for Stanford University Chronic Disease Self-Management Program.

2. Wellness and specialty certification for specific classes as needed (e.g. nutrition and health counseling).

Minimum Knowledge, Skills & Abilities Required:

1. High school diploma or equivalent; Bachelor's Degree or Master's Degree in related field preferred.

2. One (1) year of teaching experience.

3. Excellent communication (interpersonal, verbal and written) skills; speak clearly with good command of English.
