Position Purpose:General Summary:

Part of care management team that assists high risk community members and patients in navigating Holy Cross Health's care and community delivery system. Promotes improvement of health care status, through increased health awareness and improved healthy behaviors, among Holy Cross Health Center patients and broader community, by facilitating health education and wellness activities, with emphasis on importance of screening and early detection. Provides health care navigation, to connect community members to primary care, health insurance and other social services, as needed. Supports Mission of Trinity Health and Holy Cross Health.

Minimum Licensure & Certification Required (if applicable):

1. Current BLS Healthcare Provider certification from American Heart Association.

Minimum Knowledge, Skills & Abilities Required:

1. High school diploma or equivalent; Bachelor's Degree in Public Health, or related field, preferred.

2. Three to five (3-5) years of health education and outreach experience.

3. Experience in facilitating and conducting enrollment in available Federal, State and Local programs, to increase access to health care and support services.

4. Experience in developing and maintaining relationships with community agencies serving target population, and participating in community meetings.

5. Provides health education on chronic health conditions, in way that promotes understanding and self-management; facilitates screening for early detection of diseases, conducts home visits, and provides links to treatment and care.

6. Excellent oral and written communication skills; bilingual in Spanish preferred.

7. Evaluates teaching, when appropriate.

8. Experience in collecting relevant data, to measure outcomes.

9. Computer literate, with proficiency in Microsoft Office applications and internet navigation.

10. Ability to prioritize responsibilities, and adapt to changing environments, as necessary.

11. Must have transportation, to travel to patients' homes, and various Trinity Health and Holy Cross Health sites, as needed.
