Sentara Medical Group in Norfolk, Virginia is seeking a Director of Population Health Management

Responsible and accountable for the development, implementation, and performance of SMG population health initiatives, including Care Management Services, to improve quality, safety, efficiency, and patient satisfaction.

Education Level

Master's Level Degree - NURSING

Skills
Successful candidates will have:

A BSN degree and/or MSN required; NP who has functioned as a provider in the ambulatory practice setting preferred

Five years of clinical experience in a complex, integrated healthcare or payer setting. Experience in an ACO or a Clinically-Integrated Network (CIN) and with payer risk-adjusted contracts preferred.

Three years of leadership experience in a population health role, with experience in (or working closely with) Operations and IT. Experience with clinical IT platforms, and fluent with successful data analysis and interpretation.

Experience developing and implementing clinical, service, and operational process improvement initiatives; managing physician relationships; designing and using population health management resources, including care management, analytics, data, BPAs, clinical workflows.

Manages and improves diagnosis coding and documentation processes to support risk adjustment, including supervising coding teams, ensuring effective use of coding data, implementing auditing processes, and evaluating results.

Collaborates with Enterprise Analytics and Quality leaders to ensure that actionable data on population health performance is defined, obtained, interpreted, and disseminated to providers, care teams, and leaders. Develops analytic capabilities and shares key information with leaders and physicians.

Uses evidence-based clinical quality measures and predictive analytics to identify patients in need of population healthcare.

Ability to lead a team, manage resources, and budgets for a department.
Demonstrates a high degree of resilience, persistence and the ability to thrive in an environment of rapid change in multi-faceted, geographically dispersed markets.

Ability to command respect, persuade, and obtain support to accomplish the vision. Demonstrated skills in group facilitation and communication, and establishing and measuring objectives.

Ability to effectively analyze complex issues/problems and lead/influence individuals and groups in developing and implementing successful resolution tactics; ability to translate vision into a strategic action plan and execute on the plan.

Experienced and proven ability to work effectively in a highly matrix organization

Excellent presentation, communication, and organization skills
Knowledge in population health performance evaluation, including financial performance, risk assessment, and interventions to improve outcomes.

Skills in managing risk adjustment, including use of software, analytics, and best practices in diagnosis coding and documentation.

Current knowledge of regulatory requirements related to population health to ensure compliance.

Other
Bachelor's Degree in Nursing and 5 years of nursing experience including leadership responsibilities or commensurate experience to be considered. Ambulatory clinical management/administration experience required. Experience with Case/Care Management/Coordination preferred. Mgmt of Human Resources, Mgmt of Clinical Resources, including professional nurses.
