Be assigned to families in the child care option of Early Head Start.
Within 45 days of the child's entry into the program and in collaboration with the parent(s) ad the child care provider(s), assure that developmental, sensory and behavioral screening is conducted with each child.
Within 90 days, obtain medical records to indicate the presence of a “medical home”, and up-to-date well-child care including current physical exam and immunizations.
Maintain an up-to-date file on each child, providing copies of all required records to the child care center, with parental consent.
Deliver supplies and equipment to providers as needed.
Conduct a minimum of quarterly home visits with each assigned family.
Maintain regular communication between the child care setting, the family and the Social-Emotional Wellness Coordinator.
When at the child care site be of assistance to the provider.
Participate in service coordination meetings.
Member of the Child Care Committee.
Minimum CDA in Child Care, Infant & Toddlers preferred.
AA in Early Childhood Education or related field.
Minimum 2 years experience working with families and young children.
Home visiting and/or child care experience preferred.
Bilingual (english/spanish), proficient in reading, writing and speaking).
Must have reliable transportation.
Ability to work with a multi-cultural and diverse community.
Tobacco use is a well-recognized preventable cause of death in the United States and an important public health issue. In order to promote and maintain a healthy work environment, Adventist HealthCare will not hire applicants for employment who either state that they are nicotine users or who test positive for nicotine use.
