Works directly with clients enrolled in the Chronic Disease Care Management program to support the client in effective management of his/her chronic health conditions, self-care, linking with community resources and adopting healthy behaviors. Establishes trusting relationships with clients/caregivers and families while providing general support and encouragement. Provides these services in the client’s home, community, and clinic settings. Collaborates with the health care team, providers, and community partners in implementation of the client’s individualized health care plan. Provides social support, informal counseling, and advocates for individuals and community health needs, and provides services such as home visits, small group education and community outreach. Works u nder the direction of the Public Health Nurse (PHN).

Employment Standards

MINIMUM QUALIFICATIONS:
Any combination of education, experience, and training equivalent to the following:
(Click on the aforementioned link to learn how Fairfax County interprets equivalencies for “Any combination, experience, and training equivalent to”) Graduation from high school or GED issued by state department of education; plus, three years of work experience, including two years in some area of human services (related two-year

[associate] degree may be substituted for the required human service experience).

PREFERRED QUALIFICATIONS:
Experience working with diverse populations with a high volume of client contact.
Knowledge and experience with care-coordination and/or health coaching. Knowledge of the principles and practice of social work, a plus
Ability to work independently and prioritize competing tasks effectively.
Clear and effective communication skills, both orally and in writing.
Possess exceptional English/Spanish verbal and written communication skills; must pass language proficiency exam.
Work well with multi-disciplinary team members and community partners.
Ability to build and maintain positive community relationships.
Proficiency in Microsoft Office, Word and Excel applications

CERTIFICATES AND LICENSES REQUIRED:

Driver's License (Required)
AED (Required within 60 days)
CPR (Required within 60 days)

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a child protective services check, a sanction screening, and driver's license check to the satisfaction of the employer. Tuberculosis test.

PHYSICAL REQUIREMENTS:
Job is generally sedentary in nature, and job requires walking, standing, sitting, and kneeling, reaching, bending, climbing stairs; uses hands to grasp, handle, or feel. May be required to lift or carry up to 25 lbs. occasionally. Visual acuity is required to read data on a computer monitor; ability to operate keyboard driven equipment and computer. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. Generally, works in an office environment yet may occasionally be required to perform job duties outside of the typical office setting. Ability to drive a motor vehicle. All duties may be performed with or without reasonable accommodations.
