White Oak Medical Center seeks to hire a Clinical Pharmacy Specialist who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the Clinical Pharmacy Specialist in Transition of Care you will:
• Primarily function coordinating clinical pharmacy functions for patients transitioning between different levels of care in the inpatient and outpatient settings. This includes medication reconciliation from admission and discharge, overseeing Medications History Technician program, prospective medication discharge review, medication discharge counseling, and follow-up to ensure medication compliance.
• Work in collaboration with providers and other healthcare professionals to ensure the most appropriate medication management and identify potential medication related risks.
• Identify high risk patients for readmission through use of risk stratification tools (e.g. BOOST.)
• Work in conjunction with the Care Navigation team to ensure availability and affordability of prescribed medication and offer alternative treatments if potential issue is identified.
• Assist with pharmacist and/or other healthcare professional education.
• Precept pharmacy students or residents.

Qualified candidates will possess:
• Doctor of Pharmacy Degree from accredited college of pharmacy
• With ASHP accredited Residency (preferred)
• PGY-1 residency or Minimum of 3 years of clinical pharmacy experience in the area of specialty (Ambulatory Care)
• Board Certification and PGY2 Residency preferred (Ambulatory Care)
• Prior experience precepting students and residents

Work Schedule:

Monday through Friday Day shift (8 hour shift)
