Adventist HealthCare seeks to hire an experienced Clinical Practice Transformation Specialist who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Clinical Practice Transformation Specialist, you will:

• Be responsible for developing a productive relationship with the Adventist HealthCare Physician Alliance Clinically Integrated Network (CIN) practices to achieve Adventist Healthcare's strategic goals
• Provide education and guidance related to commercial payor programs utilizing advanced analytics, gaps in care files, and physician score cards to identify patients with high risk and rising risk conditions. Support practices in closing quality gaps, including patient outreach.
• Collaborate with CIN staff, care navigators, and other support teams to develop strategies and processes for managing attributed patient populations, closing quality gaps, and improving overall practice performance
• Engage CIN practices in practice transformation by serving as primary leader for developing and utilizing workflows tailored to the physician's and practice's needs.
Contribute and lead practice transformation improvement projects
• Assist in communicating key data trends for the purposes of strategy in managing risk stratification patient populations and quality metric gaps
• Participate in the Adventist HealthCare Physician Alliance CIN and its subcommittees and other required meetings
•Provide technical assistance to the practices, serving as a key resource in practice's transformation

Qualifications include:

• Bachelor's degree in Computer Science, Health Informatics, Healthcare
Administration or equivalent, MBA or MHA preferred
• 2-3 years of experience required, preferably in a healthcare setting
• Work experience in a physician practice is preferred
• Ability to work in high volume environment.

Work Schedule:

Full Time, Day Shift, Monday - Friday, 8:30am to 5:00pm

COVID-19 Vaccination

Tobacco and Drug Statement

Those testing positive are given the opportunity to re-apply in 90 days, if they can truthfully attest that they have not used any nicotine products in the past ninety (90) days and successfully pass follow-up testing. ("Nicotine products" include, but are not limited to: cigarettes, cigars, pipes, chewing tobacco, e-cigarettes, vaping products, hookah, and nicotine replacement products (e.g., nicotine gum, nicotine patches, nicotine lozenges, etc.).
