Adventist HealthCare seeks to hire an experienced Supervisor for our Home Health Intake Team at our Post-Acute Care Services Center who will embrace our mission to extend God's care through the ministry of physical, mental, and spiritual healing.

As a Supervisor, Post-Acute Care Services Center you will:
• Coordinates the education, training and support of the Post-Acute Services Center staff, to include pre-admission intake documentation into EMR systems, insurance benefit verification/authorization, patient scheduling, organization of transportation to acute rehab from acute care hospital, referral submission to post-acute services outside of the AHC network when necessary
• Supervises daily, weekly and monthly staffing/productivity trends utilizing census reports to manage patient care efficiency and effectively
• Reviews and approves time off requests, to ensure all staffing needs are met and managed.
• Collaborates with Post-Acute Care Services Manager for final staffing plans
• Identifies and leads team initiatives for process improvement related to the Post-Acute Services Center
• Orients new team members and functions as a super user
• Clearly communicates expectations to scheduling functions and handoffs/SBAR
• Performs routine Post-Acute Services Center functions and duties as needed
• Other duties as assigned

Qualifications include:
• High School Diploma or equivalent required; Associates preferred
• 3-5 years in a medical office/healthcare environment and/or Medical Office operations experience required
•1-2 years of Supervisor experience leading and coordinating staff members preferred
• LPN or RN preferred
• Knowledge and ability to perform intake, scheduling, and or/benefits verification, and review functions in EMR

Work Schedule:
  • Responsible for the m anagement of home health intake liaisons and PAC assistants.
  • Please note this is a hybrid position; remote work allowed with an in-office presence required for designated meetings and trainings
