The VISN 10 Clinical Resource Hub (CRH) is a repository of clinical and administrative staff that serve VHA facilities within a VISN. Staff within the CRH are expected to provide interprofessional services to VISN facilities that are underserved or experiencing gaps in clinician coverage due to
the inability to match provider supply with demand. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. The CRH Mobile Deployment Team (MDT) provides care through virtual, in-person, and Mobile Medical Units and are a Mission Act 402 requirement. Travel may be required for coverage to other clinics or for training. In person deployments of clinical staff for short-term assignments will be at the discretion of the V10 CRH Tele Specialty Care Chief and will depend upon clinical need and available resources. The position is a mostly virtual role and organizationally aligned under Tele Specialty Care Section, supervised by Section.
This position requires that you live within two hours of either Marion, Indiana, Fort Wayne, Indiana or Chillicothe, Ohio VA facilities as you may be required to provide direct care at one those facility.
The duties of the Nurse Practitioner - Nephrology includes but is not limited to the following:Completes History & Physicals (H&P) and review of systems to identify need for additional tests. When face-to-face care is indicated, this will enable the Nephrologist to have the necessary information and update the H&P as needed. Care is mostly completed via CVT and VVC. Orders tests at home facility such as laboratory and diagnostic studies and procedures. Assesses results of ordered tests, studies, and procedures and notifies the Veteran of results. Arranges for virtual or face-to-face visits with Nephrologist as needed.
Pay: Competitive salary, regular salary increases, potential for performance awards

occasionally be exposed to patients who are verbally combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations and able to shift priorities

- Provide clinical VISN-level coordination of Nephrology referrals between sites (initiate/answer consults, document findings, provide initial and follow up care via VVC, facilitate travel/lodging, etc.)
- Serve as the Subject Matter Expert (SME) with knowledge of specific Nephrology procedures and services completed at all sites.
- Answer e-consults and questions pertaining to findings.
- Assist with gap-coverage for consult management (internal & referrals) for V10 sites, especially those with solo Vascular providers on leave.
- Determine prior treatments attempted and refer for treatments that have not yet been attempted prior to face-to-face care.
- Screen internal consults and do all workup required:
- Provides a timely review of the medical records of patients referred to Nephrology and evaluates for red flags that indicate a need for urgent referral
- for community care.
- Initiates, responds to, and expedites requests for consultations, special tests and studies.
- Facilitates pre-operative Nephrology risk assessment when requested.
- Prescribes medications and products to patients as appropriate.
- Provides education and counseling to patients and families in preventive care, medical conditions, and use of prescribed treatments and drugs specific to Nephrology care.
- Manages ongoing treatment of follow-up patients, consulting with the Primary Care or Medicine Specialty provider as appropriate.

Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- Must have experience in Nephrology.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
- Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
- Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
- Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
- Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

- Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 3-4 years of nursing practice/experience.
- Nurse V - Master's degree in nursing or related field with BSN (or a Bachelor's degree in a related field) and approximately 5-6 years of nursing practice/experience; OR a Doctoral degree (preferred) in nursing or related field and appropriate basic or advanced certification desired, and approximately 4-5 years of nursing practice/experience.
