Provides interpretation for limited proficiency English speaking clients in a clinic, community site or home visit settings. Works closely with Public Health Nurse(s) and medical personnel to solicit health history information and communicates a plan of care to clients. Translates written materials as directed by Public Health Nurse(s) and medical personnel. Communicates results of health assessment(s) and laboratory tests to clients. Instructs clients about the need for preventative health care and early intervention. Provides culturally sensitive client/community education and health care services. Documents according to agency guidelines and polices.

Note: HSA position work schedules are on-site location and typically Monday through Friday and occasional evenings, as needed.

Required Knowledge Skills and Abilities

Knowledge of the basic principles of human behavior;
Knowledge of word processing and data management programs (Excel, survey software)
Ability to communicate effectively to clients;
Ability to organize information and records;
Ability to communicate in written format

Employment Standards

MINIMUM QUALIFICATIONS:

Any combination of education, experience and training equivalent to graduation from high school or GED issued by state department of education; plus, three years of work experience, including two years in some area of human services (related two-year [associate] degree may be substituted for the required human service experience).

CERTIFICATES AND LICENSES REQUIRED:
  • Driver's License (Required)
  • AED (Required within 60 days)
  • CPR (Required within 60 days)

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background, Child Protective Services check, a sanction screening, a driving record check, and a TB test to the satisfaction of the employer.

Candidate must pass the following:
  • Successful completion of Language Proficiency Testing
  • Successful completion of Community Health Worker formal training program
All newly hired employees are required to be fully vaccinated against COVID-19 (two weeks after the last required dose) as a condition of employment or obtain approval of a medical or religious exemption prior to their start date. Proof of an exemption or vaccination status will be required during the pre-employment onboarding process. New employees who obtain an exemption from the vaccine mandate for medical or sincerely held religious beliefs may be subject to a weekly testing requirement. Vaccinated employees and employees with a medical or religious exemption will complete the attestation online on their first day of employment or shortly thereafter.

PREFERRED QUALIFICATIONS:
Preferred candidates will be bilingual in English/Spanish. Candidates will have knowledge and experience working in a fast-paced clinical setting. Ability to reach out and maintain positive community relationships. Experience working with diverse populations. Possess exceptional verbal and written communication bilingual skills; candidates must pass language proficiency exam. Experience working as a medical interpreter in a clinical setting. Experience using a team approach for service delivery. Must have the ability to establish and maintain effective working relationships, work well with multi-disciplinary team members and community partners. Ability to exercise tact, discretion, initiative, and independent judgment. Excellent customer service skills with the ability to interact in a friendly, service-oriented and professional manner. Ability to organize information and records. Proficient experience using Microsoft Office, Word and/or Excel; data entry experience in an Electronic Medical Record system a plus.

PHYSICAL REQUIREMENTS:
Employee is required to walk, stand, sit, bend, kneel, climb stairs and carry equipment up to 25 pounds in weight as required for the position. Uses hands to grasp, handle or feel. Visual acuity is required to read data on a computer monitor. Ability to operate keyboard driven equipment. Ability to drive a vehicle. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. All duties performed with or without reasonable accommodations.

SELECTION PROCEDURE:
Panel interview and may include practical exercise.
