How you will make an impact: As Community Engagement Navigator you will be responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources. This is an exciting opportunity to impact youth and family populations in a positive way under a new program in Washington. Successful candidates will be eager to participate in building a new team and starting something new. This is a field position that will combine remote and on-site work in a variety of healthcare settings. Candidates must be in one of the counties listed in this job posting. Primary duties may include, but are not limited to: Engage with members who have been identified as possessing significant SDOH barriers. Work with the members to establish goals and next steps for addressing SDOH. Support member in selecting provider or programs most appropriate for supporting their needs. Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts. Act as a liaison between referral sources and the health plan. Minimum Requirements: BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field; A minimum of 2 years' experience with marketing, job development, job placement or social work; Or any combination of education and experience which would provide an equivalent background. Preferred Skills, Capabilities and Experiences: Experience working with youth and family population in behavioral health/ social work settings strongly preferred Candidates located in Clark County (Vancouver), Klickitat, or Skamania in Washington are strongly preferred Knowledge of Voc Rehab preferred.
