Duties
The incumbent works closely with the VA medical center's homeless programs, residential and community care programs, and domiciliary programs to ensure coordination between these entities is facilitated and veterans direct care, referral, and follow up is contiguous and based on a continuum of services. The incumbent independently provides clinical, psychosocial, and case management services at an advanced practice level for eligible veterans and their family members in the program.
Veterans eligible for HUD-VASH program have complex health and mental health needs and require a high degree of clinical oversight and creative problem solving. Services are provided in healthcare settings and in the community. The HUD-VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies,
The incumbent is responsible for data collection, administrative record keeping, monitoring patient or program clinical indicators, thresholds, and performance measurements. Additionally, the incumbent develops working relationships and agreements with other organizations and directly coordinates with the designated Public Housing Authorities (PHA) regarding housing voucher application and award process for each veteran. The incumbent is responsible to review the agreements between Section 8 landlords, PHAs, and the VA to establish appropriate referral sites for placements.
The incumbent establishes, implements, and maintains referral and screening procedures for potential HUD-VASH veteran participants that meet national policy. The incumbent screens and conducts assessments to ensure the appropriateness of placement in the HUD VASH program.
The Incumbent provides case management services to the veteran and the veterans family members that are admitted into HUD VASH. The incumbent provides supportive case management services primarily in the community and or in the veterans home.
The incumbent will develop Housing Stability plans and goals with each veteran served.
The incumbent assists veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with veteran's goals associated with healthy independent living. The incumbent refers veterans to appropriate VA and other government/community services-including local shelters, food banks, employment services, veteran's benefits, and social security applications. The incumbent assists veterans who encounter problems obtaining services from the VA and other service delivery agencies through advocacy, linking, and/or by assisting with their problem solving/social interaction skills.
The incumbent evaluates the veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability. The incumbent provides crisis intervention as clinically indicated. The incumbent effectively provides case management services while utilizing professional skill, objectivity, insight, advanced clinical training, and experience to interpret data and to identify viable treatment options and recognizing potential high risk factors, acuity, and need for services.

Basic Requirements: The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. § 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following:

Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.
Senior Social Worker, GS-12
Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:

- Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
- Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
- Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
