This is an exciting opportunity to work in a collaborative environment providing clinical and administrative supervision to a team of discharge planners who provide discharge planning services to seriously mentally ill and co-occurring adults. The primary function of the Discharge Planning team is assessing, referring, and linking hospitalized or incarcerated individuals to community-based services and resources. Involves participation in hospital treatment planning, hospital census management meetings, and staffing reviews for individuals with extraordinary discharge barriers. The supervisor assists in placement and monitoring of adults referred to programs funded by state Discharge Assistance Program (DAP) funds. Coordinates conditional release planning for NGRI (Not Guilty by Reason of Insanity) acquittees on inpatient status and provides monitoring service, including testifying in court when needed, serves as the CSB NGRI coordinator for acquittees who have been conditionally released to the community.

This position is based at the Sharon Bulova Mental Health Center in Merrifield.

  • Provides staff supervision for all clinical aspects of the program;
  • Plans, assigns, and reviews work of team members;
  • Interviews and selects candidates for various positions;
  • Performs complex evaluations and makes independent diagnoses using Diagnostic and Statistical Manual of Mental Disorders and International Statistical Classification of Diseases and Related Health Problems (ICD) - 10;
  • Provides individual, family and group treatment;
  • Maintains a caseload of diverse, complex, and high risk individuals;
  • Develops treatment plans and adapts treatment interventions and approaches;
  • Provides assessment and treatment services to individuals presenting a wide variety of emotional, social, and mental disorders over a range of severity;
  • Consults with professionals on a variety of problems including delivery of needed services for clinically complex cases;
  • Provides liaison to community agencies to facilitate communication and service provision for individuals requiring multiple services;
  • Develops and implements program activities and services, including training and educational activities;
  • Under direction, develops and implements a specialized program which is designed to maximize the effectiveness and efficiency of service provision;
  • Supervises day-to-day program operations;
  • Promotes awareness of program services through presentations;
  • Develops and monitors program's policies and procedures;
  • Ensures that program is in compliance with applicable federal, state, and local policies, regulations, and statutes;
  • Mediates and facilitates inter-and intra-program issues needing a systems perspective;
  • Plans, recommends, and executes quality initiatives related to mental health standards;
  • Monitors and analyzes the impact of psychotropic medication on an individual's functioning and mental status;
  • Provides crisis stabilization and crisis management;
  • Ensures that a facility is maintained in good operating condition and compliant with health and safety, licensure, and regulatory requirements.

Employment Standards

MINIMUM QUALIFICATIONS:
Graduation from an accredited college with at least a master's degree in social work, psychology, counseling, or nursing plus three years of clinical experience or a doctoral degree in psychology, social, counseling or nursing plus one year of clinical experience.

CERTIFICATES AND LICENSES REQUIRED:
  • Currently licensed to practice in the Commonwealth of Virginia in one of the following:
    • Licensed Clinical Social Worker,
    • Licensed Professional Counselor,
    • Clinical Psychologist,
    • Licensed Marriage and Family Therapist,
    • Licensed Substance Abuse Treatment Practitioner or
    • Clinical Nurse Specialist.
  • Valid driver's license with fewer than six demerit points (or equivalent in another state) and must be maintained throughout employment with Community Services Board.

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter. Requires a National Provider Identifier.

This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies).

All newly hired employees are required to be fully vaccinated against COVID-19 (two weeks after the last required dose) as a condition of employment or obtain approval of a medical or religious exemption prior to their start date. Proof of an exemption or vaccination status will be required during the pre-employment onboarding process. New employees who obtain an exemption from the vaccine mandate for medical or sincerely held religious beliefs may be subject to a weekly testing requirement. PREFERRED QUALIFICATIONS:
  • Experience providing discharge planning services and to high risk seriously mentally ill, co-occurring and forensic adult population.
  • Experience collaborating with multiple human services systems and state program agencies, such as jails, hospitals, ACT, and intensive case management.
  • Experience with and knowledge of community base resources and treatment services.
  • Extensive knowledge of the principles, theories, and methods of the psychological and social development of the individual;
  • Experience with and thorough knowledge of intake procedures, social history development, and interviewing techniques;
  • Experience with and ability to formulate diagnoses for clients with acute, chronic, severe mental illness;
  • Experience with and ability to diagnostically interpret data obtained from psychological test results, social histories, and interventions.
  • Prior experience delivering community based behavioral health services.
  • Experience with hospital discharges and placements
  • Experience with delivering crisis stabilization/management services to individuals with severe mental illness and/or co-occurring (MH/SA) disorders.
  • Experience working with NGRI acquittees
  • Supervisory skills at least three years

PHYSICAL REQUIREMENTS:
Ability to drive and travel to numerous locations, including some travel throughout the Commonwealth of Virginia. Ability to sit, walk, stand, bend, and write as needed to complete duties. All duties performed with or without reasonable accommodations.
