Shady Grove Medical Center seeks to hire experienced Social Workers who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Care Navigator of Behavioral Health you will:
•Complete comprehensive psychosocial assessments with a focus on patients' requirements as they transition to the next level of care
•Assess, plan, and coordinate patients' post-acute healthcare services
•Lead or participate in Interdisciplinary Rounds and other Care Conferences
•Collaborate with physicians, nurses, social workers, and therapists to foster a coordinated approach to discharge planning
•Complete required documentation in a timely and accurate manner
•Coordinate with the patients' outpatient care navigator for needed services and resources such as medication reconciliation, disease education, home safety checks, in-home discussions about care needs and challenges, social needs assessment, and preparation for follow-up appointments
•Communicate with Utilization Review staff on any denials, issues or barriers to discharge
•Obtain needed authorization for needed outpatient services
•Connect patients to community provider(s) including psychiatrists and therapists to make sure that the patient has access to prescribed medications •Assist patients to schedule medical appointments to include transportation resources

Qualifications Include:
•Master of Social Work
•2 years of social work experience (behavioral health experience preferred
•Active Maryland Social Work license
•Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance
•Active American Heart Association Basic Life Support (BLS) required
•Empathic toward individuals complex health needs and their families
•Strong communication and customer service skills
•Knowledge of community resources
•Strong writing skills
•Strong organizational skills; able to develop workflows to create more efficient processes
•Ability to work in a fast paced environment while managing multiple priorities and maintaining professionalism
•Excellent time management skills
•Ability to problem solve and exercise good judgment
