The Alexandria Health Department is seeking a highly motivated health care professional, who is passionate to make a difference in individual lives and the community, to join our team as a fulltime Social Worker to help us promote mental health across the populations we serve.


Duties include but not limited to: providing support to patients receiving prenatal care and postpartum MCH services through the BabyCare program and field case management, sexual reproductive health and other public health services. Performs assessments of the client’s situation to determine financial, social and mental health needs and goals. Will research and advocate for appropriate public assistance resources for clients, maintain communication with case management care team, provide crisis intervention, refer individuals to appropriate treatment centers, coordinate treatment planning and maintain ongoing contact with outpatient providers for the continual care of patients. Works to strengthen the mental health skills of our workforce by identifying and facilitating training opportunities. Increases AHD connections with social services programs, mental health care providers in the community to ease referrals and facilitate continuity of services. Conducts home visits for BabyCare clients that have been identified with service needs, behavioral health issues, domestic violence victims or substance use dependents or those in recovery.
