The Social Worker completes a psycho-social assessment for all patients/families; assesses needs, provides appropriate information and interventions in order to ensure that the care-giving system is stable and that end-of-life issues are addressed. Provides coordination and communication with other members of the interdisciplinary team. SUPERVISION Direct Supervisor: Clinical Manager Next Line Supervisor: Director, Clinical Services DUTIES AND RESPONSIBILITIES Assesses each patient/family admitted to Hospice of the Chesapeake (HOC) services for psychosocial issues impacting on the care-giving system, including an assessment of the environmental, age specific, spiritual and financial impact of loss on patients, families, caregivers, and clients. Formulates individualized plans of care for patients based on assessed needs, strengths, limitations and goals. Includes patient and caregivers in the care planning process. Ensures that patient's desires are taken into account when the treatment plan is formulated. Regularly reassesses for changing needs. Participates actively in IDT meetings, functioning as an advocate for the patient and family. Communicates initial/ongoing plan of care appropriately to multidisciplinary treatment team. Collaborates with providers and treatment team to identify and evaluate patient outcomes and to evaluate variances in expected behavior and appropriateness of care. Contributes toward identifying social problems, their severity, and interrelatedness to the medical situation, in addition to assessing caregivers' strengths/weaknesses with focus upon methods to alleviate the situation. Maintains current referral information on local human services organizations and provides timely referrals to patient/caregivers as needed. Provides follow up as appropriate to assess effectiveness, and assumes an advocacy role, when indicated, to facilitate holistic care. Demonstrates understanding of Medicare/Medicaid, and other funding sources. Provides counseling and emotional support and/or crisis intervention as per professional standards. Offers support during/after crises in coping with the psychosocial stress precipitated by illness, hospitalization, loss, disability, and death by implementing problem solving and supportive care which will work to reduce the patient's and family's emotional discomfort and facilitate adjustment to present life situation. Engages with the patient/caregivers to develop a rapport and a sense of trust in the care being given. Uses a variety of interviewing techniques to provide supportive counseling as well as identify underlying issues. Consistently demonstrates empathy with patients/caregivers. Reports any suspicions of abuse and/or neglect of a patient to appropriate authorities. Attends staff meetings and mandatory in-services as per department requirements. Reads minutes and demonstrates knowledge of content. Makes arrangements with appropriate supervisor for satisfactory compliance when unable to attend. Submits accurate timesheets and correctly documented logs to Clinical Manager by scheduled deadline. Demonstrates problem-solving skills with patients, their family, employees, other departments, visitors and others as indicated. Maintains open and professional communications with all staff. Intercedes as necessary to promote positive, constructive interactions between staff members/departments/others. Reports issues to appropriate supervisor. Develops an effective working relationship with representatives from other agencies who provide services for assigned patients. Makes community referrals appropriately. Demonstrates responsibility for scope of position/own standard of practice. Maintains required skills, licensure, regulatory requirements and credentials needed to perform assigned duties. Demonstrates full knowledge of current position's, and department's, relationship to flow of patient services and/or care plan of the patient. Demonstrates understanding of all other services of the organization. Participates in and supports continuous process improvements. Other duties as assigned. QUALIFICATIONS Master Social Work degree from an accredited educational institution required. LGSW required. LCSW-C preferred. 3-5 years in health care field preferred. Prior homecare, hospice experience or case management preferred. Demonstrated excellent oral and written communication skills; listening skills. Demonstrated ability to work effectively independently and as a member of a team Demonstrated emotional maturity and the ability to cope and work effectively with stress of continual loss. Exhibited understanding of, and acceptance of, the hospice philosophy. Well-developed critical thinking skills and problem-solving skills. Basic knowledge and understanding of medical terminology. Ability to communicate effectively with medical professionals of all disciplines. Basic computer skills, to include but not limited to Microsoft Word and Outlook, required. Ability to prioritize caseload and issues. Ability to simultaneously handle multiple assignments and projects with attention to details, efficiency and accuracy deadlines. REQUIREMENTS Must have a valid driver's license and a good driving record. Compliant with health screening and vaccination requirements of organization and position, including TB screening and Influenza vaccination annually.
