Job Description Position: Rehab Case Specialist Department: Psychiatric Rehabilitation Program (PRP) Location: Main Office Charles Office St. Mary's Office Reports To: Rehabilitation Program Coordinator FLSA Status: Non-Exempt Program Overview: The Psychiatric Rehabilitation Program (PRP) provides services to both adults and children in the community who have been diagnosed with a mental illness. The PRP program assists individuals in maintaining their independence and helps them to remain an active part of their community. The Rehab Case Specialist (RCS) will help the individuals on his/her caseload to work towards their own personal mental health goals. The RCS will assist these individuals in finding and utilizing community resources such as food pantries, shelters, churches, financial assistance, medical assistance, disability income, food stamps, therapy, psychiatry, somatic doctor's, energy assistance, housing, etc. The goal of the PRP program is helping individuals to meet their personal definition of recovery. General Description: The Rehab Case Specialist provides assistance to individuals in utilizing and reconnecting them to their natural supports in the community. The RCS collaborates with community resources to ensure both the mental and physical health needs of the individual are being met. The Rehab Case Specialist reports to the Rehabilitation Program Coordinator. Hours of employment are varied depending upon the needs of the consumers. The RCS may be required to work evenings and weekend hours to assist the consumers on his/her caseload. Qualifications: A High School Diploma/GED is required, preferably supplemented by other course work or work experience in psychology, mental health or related fields. Bachelor's Degree in Social work, Psychology, Rehabilitation Counseling, or related disciplines is preferred. Additional experience must consist of at least one (1) year in direct service. Relevant work experience within the human services field may be substituted for the above qualifications. Job Knowledge, Skills and Abilities: Possess a clean driving record with no more then 2 points. Must be insurable under SMCN's vehicle coverage. Ability to pass a criminal background investigation and drug screening. Demonstrated understanding of and willingness to participate in a team environment. Demonstrated ability to collaborate with all employees, persons served and community agencies. Excellent verbal and written communication skills are required. Must also have working knowledge of Microsoft computer programs and the ability to use the internet for business purposes. Work Environment: While performing the duties of this job, the employee is exposed to natural weather conditions as this is a mobile job and frequently requires traveling and community visits. The noise level in the work environment is usually moderate. Essential Duties and Responsibilities may include the following, but are not limited to: Collaborates with consumers to promote and develop partnerships with natural and community supports. Provides linkages and access to the necessary resources, entitlements and supports to assure successful community integration. Develops Individual Rehabilitation Plans (Treatment Plans) to identify with persons served their barriers to community integration and develop strategies to offset them in consultation with individual and their supports to facilitate both development and implementation. Documents visits with persons served in encounter notes, while performing annual assessments and monthly progress notes accurately and timely. Assures that the assigned cases and all documentation are in compliance and accurate. Adheres to good ethical practices and maintain professional boundaries with consumers, community resources and co-workers alike. Schedules, coordinates and reviews individual rehabilitation plans (treatment plans) within 30 days of admission and regularly thereafter, in accordance with the persons served needs and COMAR regulations. Assists in planning and coordination of all rehabilitation activities including self-care skills, medication monitoring, health promotion, safety, communicable disease prevention, crisis services and other areas as needed with persons served. Performs crisis intervention as needed and seeks professional help depending on the severity of the situation. Encourages consumers that are able, available and interested in working to explore opportunities with the supported employment program. Report any suspected or actual incidents of abuse and neglect to the appropriate authority/agency. Promotes Southern Maryland Community Network services while developing and maintaining a positive relationship with individuals, groups, and businesses in the community at all times. Confers and advocates with community resources serving the mentally ill including psychiatrists, therapist, mental health agencies and legal services that will promote the person served and their well-being. Assist consumers in completing applications for federal, state and local assistance programs for which they might be eligible for. Assists individuals to plan, make informed choices, follow through on responsibilities, and advocate for the person served when needed on the decisions made. Demonstrates the ability to communicate clearly and concisely both verbally and written. Employees are expected to demonstrate cultural awareness by not discriminating against persons served or co-workers based on their age, gender, race, religious preferences, sexual orientation, etc. Employees are expected to abide by SMCN's Code of Ethics and report any ethical violations to Human Resources as per policy. Works within the provisions of HIPAA and SMCN's confidentially agreements. Participate in clinical coordination meetings and provide active participation with professional and applicable information as needed to identify and collaborate with other SMCN departments for the well being of the persons served progress. Provide prompt, dependable and courteous transportation services to persons served as needed to appointments and community outings. Maintains company vehicles and reports and unusual occurrences and immediately informs the appropriate authority of any incidents, accidents or deficiencies. Properly uses all technology including computers, laptops, the internet, phones, and fax machines within the confines of the SMCN Employee Handbook. Promotes a positive work environment within the rehabilitation department and the other departments with in SMCN. Attends all meetings, trainings as scheduled and directed. Ensures that all policies and procedures set by SMCN and individual programs are up held and enforced. Will ensure that his/her CPR, First Aid and annual privileging training is up to date. Employees are expected to contribute to agency fundraisers by volunteering at events or sitting on the Development Committee. Employees are expected to help SMCN create a healthy and safe work environment by participating on the Health and Safety Committee. All other duties as assigned by the Rehabilitation Program Coordinator.
