Our Northeast Ohio Visits office services our adult and geriatric clients on a per visit basis in territories throughout Cuyahoga Falls and surrounding areas. We are looking for a Clinical Associate who will split their time between the office and field. This is a Full Time salaried position with growth potential You may spend 50% of the time: Following a designated care plan in accordance with patient's needs Making home visits to clients in designated geographic territories Performing assigned duties, including administration of medication, wound care, treatments, and procedures Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager Following up with, execute, and properly document doctors' orders Performing client assessments as necessary Case management and coordination Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet. The other 50% of the time, you will be: Using your clinical expertise and mentoring skills to lead, inspire, and motivate Home Health Clinicians Work collaboratively with the Clinical Educator to assist with new hire training and orientation. Serve as a mentor and resource to Home Health Field Clinicians.
