Location 692 - ProMedica Skilled Nursing and Rehabilitation - Brightwood, Maryland Educational Requirements Centers with more than 120 Beds: Bachelors in Social Work or a Bachelor's in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology; Licensed where required by law. Centers with 120 Beds or less: High School Diploma required; Bachelors in Social Work or a Human Services field desirable; Licensed where required by law. Position Requirements For Centers with more than 120 Beds: One year of supervised Social Work experience in a health care setting working directly with geriatric individuals. For Centers with 120 Beds or less: Two years of supervised Social Work experience in a health care setting working directly with geriatric individuals desirable. Job Specific Details: Our high volume short stay facility is looking for an energetic addition to the Social Services family! This position involves gathering resident assessment details and coordinating discharge planning needs with our downstream providers. Previous discharge planning experience required in a skilled nursing facility or hospital setting .
