POSITION SUMMARY: Directs the work of outpatient professional Clinical Social Workers and Community Health Workers as well as the administrative tasks to facilitate operations of Community Initiatives programs in support of Population Health Essential Functions: Management: Assists Director of Population Health with organizing, developing and coordinating services and activities provided according to the needs of the organization. Provide program and fiscal oversight to multiple community grants Write grant reports as required by funder Assist program coordinators with their grant management responsibilities Collaborate with Grants Dept to respond to and submit grant proposals Collect data to evaluate program impact. Supervision: Provides clinical supervision to various levels of staff to ensure proper service delivery. Guides program coordinators/ supervisors in management of their staff. Provide direct clinical supervision to clinic based social workers and community health workers Identify staff development and training needs and ensure training is obtained Development/Consultation: Works with Director of Population Health to establish new program protocols while representing the roles of social work and community health workers. Participate in mobilizing resources in the community and maintain cooperative working relations with community partners Provide guidance to staff as new protocols are written and implemented Facilitate communication among internal and external systems to institute necessary changes Promote conditions that encourage respect for cultural and social diversity through practices that demonstrate respect for difference and confirm equity and social justice for all clients QUALIFICATIONS AND REQUIREMENTS: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline Education/Discipline: Social Work 7+ years LCSW-C required Location: LifeBridge Health .
