Adventist HealthCare is hiring a full-time Care Navigator in Rockville, Maryland. We are currently offering a $5,000 sign-on bonus to qualified candidates!

We're seeking an experienced Care Navigator who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the Inpatient Care Navigator, you will:
•Identify and coordinate patient care needs in anticipation of the next site of care after hospitalization
•Provide guidance for clinical coordination, and collaboration among multidisciplinary caregivers
•Complete comprehensive psychosocial assessment and review clinical needs of patients as they transition to the next level of care
•Collaborate with physicians and social workers to foster a coordinated approach to discharge planning
•Utilize clinical knowledge to participate in treatment plan discussion with clinical teams
•Identify and connect patient to services and resources available to them
•Communicate with Utilization Review staff on any denials, issues or barriers to discharge
•Document assessments and interventions according to departmental standards
•Participate in process improvement activities

Qualifications Include:
•Bachelor of Science in Nursing or Master of Social Work
•Minimum of 3 years experience in diverse clinical settings
•Licensed in the State of Maryland
•Active American Heart Association Basic Life Support (BLS) certification required
•Prior case management or discharge planning experience preferred
•Prior experience with Cerner EMR and All Scripts is preferred
•Working knowledge of federal, state and local laws that govern health care and case management
•Knowledge of community resources
