This position functions to ensure that patients who are referred for behavioral health services will be linked to appropriate services either at the time of initial referral or throughout a patient's active involvement with CTO BH services. This position has direct patient contact to coordinate referrals and schedule appointments to community treatment and support services that have been identified by the Health Behavior Specialist. The position also works with the HBS to optimize treatment planning, providing up to date information on community resources. The position assists with patient navigation through the healthcare system, tracking successful engagement with identified services/programs. Maintains a behavioral health resource list and has a working knowledge of programs and services throughout the behavioral health system of care. Provides information to and from both internal and external facilities/resources to facilitate patient navigation in the community. At the direction of the HBS, identifies resources that support the patient's treatment plan and facilitates referrals and arranges appointments with identified programs/services. At the direction of the HBS, outreaches and engages with patients to involve patients in process of referral to community services. Provides follow up to determine successful linkage to identified services Engaged in ongoing care coordination with CTRC HBS or site assigned HBS as indicated. Works in collaboration with the Behavioral Health Team and Care Management team to identify barriers to successful linkage to community services and develops a plan to mitigate barriers. Completes documentation and maintains tracking of services as required.Department Preferred Education: Associate Degree Required, Bachelor's degree in behavioral health discipline preferred. Experience may substitute for degree requirement.Department Preferred Experience: Minimum of three (3) years related experience in the health field, with preferred work experience in behavioral health, as a referral coordinator, community health worker, peer counselor or member of an intake team.
