Home Health Program Manager POSITION SUMMARY : The Program Manager works with the home health team ( Director of Clinical Services, Home Health Clinical Manager and Home Health field clinicians) to coordinate conditions for delivery of services that reflect the Agency's philosophy and the standards of health and nursing care for people receiving services. The Program Manager shall report to the Home Health Director of Clinical Services. QUALIFICATIONS : * Have earned a High School Diploma or equivalent. * Have a minimum of three (3) years of successful experience in a related administrative assistant or office position. * Demonstrate excellent interpersonal skills, general bookkeeping skills, organizational and filing skills, and the ability to use electronic equipment for word processing, data management, information retrieval, visual presentation, and telecommunications. * Be proficient in the use of Microsoft Office or an equivalent computer program. * Demonstrate the ability to communicate effectively in English, both orally and in writing, using proper grammar and vocabulary. * Demonstrate appropriate telephone etiquette with proper voice inflection. * Demonstrate the ability to maintain confidentiality. * Not been determined to be: * On the Medicaid exclusion list; * Convicted of a crime of violence in violation of Criminal Law Article 14-101, annotated code of MD * In violation of Health-General Article 7-1102 annotated code of MD * A person with a criminal history which would indicate behavior potentially harmful to people receiving services documented either through a criminal history records check or a criminal background check * The Program Manager must have on record all required employment documents. Additionally he/she must have all other documents listed below. * Application form * Maryland driver's license and automobile insurance. * CPR card. * Health clearance/Physical within 12 months. * Criminal background clearance * Reliable transportation. * Good written and verbal communication skills. * I-9 Documentation. * Social Security card. * W-4 Federal Income Tax Forms. * State Tax Forms. * TB or Chest X-Ray within 12 months. * Hepatitis B series/declination. * At least three (3) letters of reference (must be in good standing). DUTIES : * Attend daily skilled meetings and document all critical information and updates for current patients. * Make daily COVID -19 calls to clients receiving services by OHC clinicians to obtain any information required for daily visits and log appropriately. * Approving EVV visits in EMR daily. * Complete new admission scheduling for the HHA in EMR for all new admissions. The Home Health Program Manager is responsible for following the Home Health Scheduling Process when a new SOC is sent from Intake. * Follow up and track daily physicians orders, POCs, for all patients to ensure accuracy and timely program deadlines are met. If the Program Manager finds during the orders management process that any order is approaching the required deadline of 14 days, it is their responsibility to escalate the urgency to the agency. * Participate and help in coordinating the staff meeting. * Check efax on a 15 minute interval through the day. Send faxes through OHC fax email to appropriate recipients as needed and upload client documents into the EMR. * Other tasks assigned by the Clinical Director/Clinical Manager. * Conducts a daily sweep of all incoming and outgoing documents in the EMR and updates logs accordingly. * Maintains and update POC tracking and initiate up escalation to designated staff when appropriate WORKING CONDITIONS : * Work involves exposure to uncomfortable and unpleasant surroundings. * Work an essential employee; this may occur when called upon to assist with natural disasters and or public health emergencies. * Work may involve exposure to hazardous conditions which may result in injury. * Work with terminally ill persons.
