Care Navigators are committed to removing the member's barriers to care by identifying needed resources, helping them navigate through health care services and systems, and promoting holistic health through education. Telephonic communication with individuals who need assistance navigating care for a mental health or substance use disorder to ensure access to the right care at the right time (within benefits and networks)Complete member screening tool to understand their illness/treatment plans, identify intervenable needs and coordinate resources to meet psychosocial needs, connecting members to internal and external programs when needed. Care Navigators do not make independent clinical judgements, but can educate about behavioral health issues, and follow up to make sure members kept appointmentsAs the member's advocate, work with the Employee Assistance Program (EAP) vendor to identify treatment programs that are evidence based for member transition, supporting engagement at the right level of care.Provide condition education, support, and assistance as an individual or family engages in treatmentConsistently accurate and professional documentation; meet metric and auditing benchmarksPotentially cross trains to support other contracts within KEPRO to provide a flexible workforce and meet client/customer needs. What You'll NeedRequired QualificationsRequired: Bachelors in Social Work, Counseling, Psychology, or other closely related healthcare field with at least 1 year experience in medical, mental health and/or behavioral healthcare, social work, community outreach, or patient advocacy Preferred: Masters in Social Work, Counseling, Psychology, or other closely related healthcare field with at least 1 year experience in medical, mental health and/or behavioral healthcare, social work, community outreach, or patient advocacyPreferred but not required: LCSW, LCPC, LPCC, LMFT Knowledge, Skills, AbilitiesMust be able and comfortable with maintaining metrics and goals within the departmentMust be comfortable working telephonically and with computer systems while assisting membersMust be comfortable making outreach to members without prior engagementExcellent verbal/written communication skills required.Ability to work in a team environment.Flexibility and strong organizational skills needed.Must be proficient in MS Office and internet/web navigationExperience1 year experience in medical, mental health and/or behavioral healthcare, social work, community outreach, or patient advocacy requiredStrong understanding of cultural competency with working populationExperience with commercial health plans desirable but not requiredBi-lingual (English/Spanish) desirable but not requiredThank You!We know your time is valuable and we thank you in advance for applying for this position.
