Position Title: Licensed Practical Nurse (LPN)

Basic Function: Under the supervision of the Registered Nurse, the incumbent provides health care services to individuals served by My Own Place, Inc. in accordance with My Own Place, Inc. policies, licensing regulations and principles of normalization.

 

The Specific Responsibilities:

1. Provides direct nursing care to include first-aid treatment.

2. Supervises and schedules medication nurses. Administers medication in the absence of a medication nurse.

3. Completes monthly, nursing reviews for individuals residing in the District of Columbia cosigned by the RN.

4. Ensures medical appointments are made and follow-up visits are performed per doctor=s orders. Assists with medical appointments as needed or identified by supervisor.

5. Ensures bi-annual re-certification by Primary Care Physician.

6. Reviews laboratory results and informs physician immediately of any abnormalities.

7. Notifies physician of all changes, admissions and discharges and ensures that the physician completes a follow-up assessment.

8. Reviews physician orders on a quarterly basis and contacts pharmacist to update/change existing order sheets.

9. Develops nursing care plans.

10. Meets with the QIDP and Program Manager monthly to review medical records, medical appointment needs and IHP recommendations. Provides the director with a summary of medical concerns.

11. Conducts quarterly visits to all homes to observe self-medication programs and documents observations.

12. Serves as a medical resource for staff on issues pertaining to the individuals served.

13. Attends monthly psychotropic medication reviews.

14. Provides in-service training for new staff on a regular basis as well as other clinical/health-related in-service training when directed.

15. Returns all non-schedule II medication to designated pharmacy as needed.

16. Ensures adequate inventory of medical supplies, medications in each residence.

17. Meets with pharmacist on quarterly basis to review issues.

18. Performs other nursing duties as assigned.

Internal Relationships:

Has regular contact with individuals served nursing staff and residential staff. Has occasional contact with other agency staff.

External Relationships:

Has ongoing contact with the physicians, DDS nursing staff. Has frequent contact with pharmacy.

Qualifications:

Must be a graduate from an accredited nursing program and maintain current Board of Nurses licensure in the District of Columbia. Must be certified in CPR. Must have a general knowledge of specific needs of persons with disabilities and must be familiar with Medicaid and District of Columbia regulations.
