White Oak Medical Center seeks to hire an experienced Social Worker who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Social Worker you will:
•Collaborate with other team members to include the following: family meetings, provide education regarding MOLST, and completing of Advanced Directives
•Screen patients to identify psychosocial needs/acuity and opportunity for SW intervention based on case finding, patient needs, and referrals from care team at admission
•Consistently interpret social, emotional, and family problems to attending physicians and other team members via rounds, and documents as often as indicated by cases
•Assist with discharge planning which includes collaboration with patients/families, team members, and community agencies in a concise and
•Educate patient and family on the intent of the intervention as needed
•Maintain acceptable working knowledge of resources needed to provide patients with continuity of care before and after discharge
•Regularly update knowledge base on subjects regarding State (Medicaid), Federal (Medicare) requirements and the regulations/resources needed to provide patient with continuity of care
•Contact Managers to discuss new or unresolved barriers, to include budgetary issues, patient advocacy, and policy clarification
•Act as a liaison between the hospital and outside agencies/legal authorities and referral sources
•Document all activities relevant to casework, discharge planning and follow-up in accordance with AHC Policy, Procedure and Guidelines and regulatory requirements

Qualifications Include:
•Master's degree in Social Work from a CSWE accredited social work program required.
•3 years' experience post master's required
•Licensed to practice as a Licensed Clinical Social Worker in Maryland (LCSW) or applicants with an active license from another state must obtain Maryland LCSW within 18 months of hire date
•Prefer SW certified in Palliative Care
