Works collaboratively within the Clinical Department and with the entire program to provide timely, proficient, patient-centered services for all patients and supporting physicians’ treatment plan. Collaborate with physician and multidisciplinary team members providing physical and psychological support to patients.

 

Mandatory Minimum Requirements:

 

  1. Current certification as a Nurse Practitioner or Physician Assistant Current BLS or ACLS certification Minimum four years experience in an area of women’s health and/or experience in infertility specialty. Ability to access, input and retrieve information from a computer Knowledge and proficiency of all office based procedures (clinical-both GYN and all aspects of Infertility) Knowledge of office procedures and machines (i.e. – computer, fax, copier) Ability to work as part of a multidisciplinary team Ability to remain calm and respond appropriately to all stressful situations Ability to communicate accurately and concisely, all patient instructions and those to fellow employees Excellent interpersonal skills Ability to multi-task and work well under daily pressures Ability to interact with Clinical Staff Manager, Practice Providers, practice patients, co-workers, and other management within the practice, in a professional and personable manner. Ability to work weekends during IVF Up Time (as needed) Basic Clinical and Technical Responsibilities
    1. Perform routine in-office examinations and procedures as necessary. Perform routine patient assessments as necessary. Perform phlebotomy and injection duties. Perform medication administrations. Perform patient medication teaching as needed. Documents patient care services by charting in patient and department records. Participate in daily meeting for review of patient results and instructions. Perform patient call-backs with test results and/or instructions, as needed. Maintains safe and clean working environment by:
      1. Complying with procedures, rules and regulations, and calling for assistance from other healthcare support personnel when appropriate. Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations. Operate specialty equipment per manufacturer’s recommendations and/or policy and procedure. Clean and maintain instruments and equipment per protocol and according to OSHA guidelines. Performs routine maintenance and acts as liaison with the company representative. Stock’s examination rooms with required supplies as necessary.

 

 

Essential Clinical Job Functions or Abilities

  1. Assist and chaperone physician and/or mid-level provider with procedures such as:
    1. Hysterosalpingogram Office Hysteroscopy Sonohysterogram Ultrasounds U.I. GYN Exams Endometrial biopsies Cyst Aspiration All other as trained and required by Providers
    Measure patient vital signs (weight, height, blood pressure, pulse, respirations, temperature) Assist with patient education Manage patient flow and room turnover & inform provider of schedule changes Prep exam rooms for patient according to procedure to be performed Perform phlebotomy duties and injections Gather new patient and consultation patient information and present to the Provider Perform clerical duties to maintain effective office operations as required Provide support to the Provider by anticipating their needs

 

 

Essential IVF Clinical & Other Job Functions

 

     *Training will be provided by Clinical Team for any procedures not currently proficient.

 

 

 

  1. Counsel patients regarding their treatment per the Provider Coordinate and implement patient teaching for IVF. Arrange/coordinate epididymal sperm aspirations or testicular biopsy procedures with urology specialists (as needed) Set-up room with appropriate equipment for epididymal sperm aspirations or testicular biopsies (as needed) Review patient chart to identify potential medical problems, which require evaluation before proceeding to IVF and bring to attention of Provider. Review patient chart(s) to ensure that all prerequisite lab work and tests have been completed and are within normal limits. IVF Testing Spreadsheet – work with IVF team to development monthly spreadsheet for each IVF series to include all patients/recipients and donors, and log all tests completed in preparation for cycle. Is to be prepared in advance of monthly IVF team meeting, and contact patients to schedule any missing or needed testing prior to stimulation (as needed) Verify all testing is completed prior to onset of IVF stimulation Order outpatient medications for patients Prepare the stimulation calendar for IVF, and F.E.T patients for each series. Make appointments for IVF Baselines appointments, and endometrial biopsies, and mail patient packet with all instructions (includes medication instructions, consent forms, etc.) Advise Financial Supervisor and/or Clinical Manager of any baseline appointments scheduled for outside of the designated dates (to ensure IVF letter is drafted and mailed to patient). Compile list of all needed clinical pre-IVF procedures and forward to Scheduling staff to contact patients with appropriate appointments (i.e. Endo Biopsies, Sono-HSG, cultures, EKG, chest X-ray, etc.). Conduct Medication Training Classes (as directed by Clinical Supervisor and part of the IVF team) for all current IVF patients and Donors. Perform hCG instructions with patients according to Provider instructions, and coordinate times with Clinical Manager. Accountable for accurate and timely communication of laboratory results for patients on phone tree or by direct communication with the patient (as needed). Responsible for necessary knowledge and appropriate licensure, to assist in the IVF/OR area and recovery room. Assist Providers during IVF Ultrasounds, oocyte retrievals, and embryo transfers (as needed) Perform patient “cleanings” prior to start of oocyte retrieval case (as needed) Assist during testicular biopsy procedures (as needed) Perform needed services for OHSS IVF patients requiring serum albumin (as needed) Set-up procedures rooms for oocyte retrievals and embryo transfers (as needed) Assist in Recovery Room for oocyte retrieval and embryo transfer patients (as needed) Provide post retrieval and post transfer instructions to patients (as needed) Escort post retrieval and post transfer patients to the Financial department for collection of any additional fees (as needed). ABILITY to assist in ordering anesthesia medications for upcoming series, and responsible for maintaining security of all, and maintain medication logs of all received and used drugs. ABILITY to review/verify inventory of anesthesia medications with Anesthesiologist each day prior to 1st retrieval case start.

 

 

THIRD PARTY REPRODUCTION DUTIES: Will be trained for the following

 

Essential & Specific Donor Egg Coordinator Job Functions 

 

 

 

  1. ASSIST WITH- Internet based donor screening program ASSIST WITH- Pull all applications from program, contact all initially qualified potential donors to complete in depth screening form ASSIST WITH- Call to schedule initial appointment for all donors qualified based on “long form” ASSIST WITH- Schedule Psychological evaluation for both Donors & Recipients as needed, and ensure receipt and qualifications are met ASSIST WITH- Schedule all FDA & Program screening for Donors needed, and ensure complete as part of the acceptance process ASSIST WITH- All qualified Donors information/profile to be developed and added to the Donor Program System ASSIST WITH- Ensure all appropriate consents forms for Donor Program are properly executed upon entry into the donor program (Donor SPECIFICALLY) ASSIST WITH- Contact potential Recipients (per MD) and provide donor profiles for review and matching purposes ASSIST WITH- Coordinate cycles/protocols for Donors and Recipients, according to Provider direction. ASSIST WITH- Ensure all appropriate Stimulation Cycle Consent forms are properly executed by both the donor and the recipient ASSIST WITH- Manage all Third Party Reproduction (i.e. Coordinator) ASSIST WITH- Maintain records of egg donation while maintaining donor and recipient anonymity. ASSIST WITH- Manage program in accordance with NHC SOP’s and within all FDA guidelines and regulations ASSIST WITH- Discuss any violations of SOP’s with both the Clinical Staff Manager and Medical Director ASSIST WITH- Order medications for both Donor and Recipient once protocol complete and Series chosen ASSIST WITH- Contact Recipient couple with appointment times for day of Donor’s oocyte retrieval (i.e. female-cathfitting, male-sperm collection or TESE procedure) ASSIST WITH- Contact Urology to coordinate any/all TESE procedures for donor egg recipient male partner ASSIST WITH- Participate in all potential new hire for any clinical staff needed to assist with Program (with Clinical Staff Manager and/or Administrator).

 

 

Essential & Specific Gestational Carrier and Donor Embryo Job Functions 

 

 

 

  1. ASSIST WITH- Internet based Gestational Carrier Program
    1. Pull applications from program, contact all initially qualified potential gestational carriers to complete in depth screening form Call to schedule initial appointment for all gestational carriers qualified based on “long form” Schedule Psychological evaluation for both Gestational Carrier & Recipient/Intended Parents as needed, and ensure receipt and qualification are met Schedule all FDA & Program screening for Gestational Carrier as needed, and ensure complete as part of the acceptance process All qualified gestational carrier information/profile to be developed and added to the Program System Ensure all appropriate consents forms for Gestational Carrier Program are properly executed upon entry into the program (Gestational Carrier-SPECIFICALLY) Contact potential Recipients/Intended Parents (per MD) and provide Gestational Carrier profiles for review and matching purposes Coordinate cycles/protocols for Gestational Carrier and Recipients/Intended Parents (to include a Donor if applicable), according to Provider direction. Ensure all appropriate Stimulation Cycle Consent forms are properly executed by all parties (Gestational Carrier, Recipient/Intended Parent, and Donor if applicable)
    Maintain records of program Manage program in accordance with NHC SOP’s and within all FDA guidelines and regulations Discuss any violations of SOP’s with both the Clinical Staff Manager and Medical Director Order medications for all parties involved in the cycle, once protocol complete and Series chosen Contact Recipient couple with appointment times for day of Gestational Carrier’s appointment (i.e. Embryo Transfer) Contact Urology to coordinate any/all TESE procedures for recipient male partner (as needed) Participate in all potential new hire for any clinical staff needed to assist with Program (with Clinical Staff Manager and/or Administrator). Any/All other in accordance with this program and per Medical Director

 

 

  1. Manage: All aspects of the Donor Embryo Program 
    1. Ensure all screening completed (per FDA) on donating couple Develop donor embryo profile for program, based on donating couple’s characteristics Meet with potential Recipient(s) and provide profiles for review Upon selection of a profile/set of donated embryos, forward information to Financial office to meet with Recipient(s) to sign all proper consents and handle all financial responsibilities Ensure all proper consents (clinical) for program and executed properly with Recipient(s) Ensure all appropriate FDA screening is scheduled and performed by Recipient(s) Order all medications for Recipient, once protocol completed and Series chosen Any/All other in accordance with this program and per Medical Director

 

 

Additional Responsibilities

 

  1. Ensure that appropriate paperwork and consent forms have been received by/for patient treatment Attend meetings as assigned and implement any changes to protocols to supervisees and other clinical staff, and oversee their adherence to the changes. Cooperate and work independently to ensure Practice goals are achieved Ensure confidentiality and all HIPAA guidelines are observed for patients and their records Cooperate and work to ensure Practice goals are achieved Participate in any/all trainings provided by Practice, and intended to improve current abilities or in learning new abilities as required by the Practice. Participate in any/all goal plans as implemented by Clinical Staff Manager and/or Providers Participate in any/all proficiency and/or goal improvement plans as implemented by Clinical Staff Manager Participate in any/all proficiency testing as required by the Practice, understanding satisfactory results of testing are required to maintain position with the Practice
